What this site collects, and what happens to it
Plain version first: your six answers are never sent anywhere. The only thing that collects anything is the optional tour-inquiry form after your result, it asks for no health information, and its purpose is to pass your name, contact details and the care level those questions suggested to Westminster Place Admissions so they can contact you about a tour.
The assessment collects nothing
The six questions run entirely in your browser. Your answers are never sent anywhere, never stored, and never attached to your name. Close the tab and they are gone. The server refuses a submission outright if any answer field is present, so this is enforced rather than promised. What does travel, if you use the form, is the care level those answers produced — one label out of six, such as “Assisted Living”. The reasoning behind it does not.
What the tour-inquiry form collects
If you ask to be contacted about a tour, this is the complete list:
- First name and last name
- Phone number, email address, or both — whichever you give
- The date and time you submitted, generated by the server
- A unique reference number, generated by the server
- The care level the six questions produced — one of six fixed labels, never your individual answers
- The exact permission wording you were shown when you ticked the box, stored word for word alongside your record
That is everything. No individual assessment answers. No diagnosis, medications, medical history or date of birth — none of which were ever asked for. The server rejects a submission outright if any field of that kind is present, rather than accepting it and discarding it later. The care level is the single derived value that is kept, and it is kept so the right Admissions team is the one that calls you.
Why it is collected
For two purposes, and no others. First, so your details can be passed to Westminster Place Admissions and they can contact you about a tour. Second, so there is a dated record that your inquiry came through this guide.
It does not schedule anything and it does not guarantee a tour — Admissions decide that. It does not put you on a mailing list; there is no mailing list. Nobody is paid for your inquiry.
Who operates this site
This guide was independently developed by an employee of Westminster Place. It is not an official Westminster Place website. No referral fees are received from Westminster Place or anyone else.
Where it is stored, and who else touches it
Two outside companies are involved, and no others:
- Netlify hosts the site and stores the submission records. Records are held in a private store that is not reachable from the public internet.
- Resend delivers the notification email that tells the site operator a submission arrived. That email contains your name, contact details and care level, so Resend processes and retains those under their own terms, which this site does not control.
- Your request itself reaches Netlify's servers, which record standard technical details such as the time and the response status. Those logs contain the submission reference and whether the save succeeded. They do not contain your name, email or phone number.
There is no analytics service, no advertising network, no tracking pixel and no cookie. The only outside request the page itself makes is for its typeface.
Sharing with Westminster Place
Your information is not shared with Westminster Place unless you ask for it to be, by ticking the permission box on the form. The box is unticked by default and the form cannot be submitted without it, because sharing is the whole point of submitting it. If you do not want your details passed on, do not use this form. Westminster Place publishes its own contact details and you can reach them without this site at all — their general number is (847) 570-3422 and their website is presbyterianliving.org. Nothing is lost by going direct; this guide keeps no record of you if you do.
If you do tick it, here is exactly what happens. Your first name, last name, phone number, email address and the care level the questions suggested are given to Westminster Place Admissions so they can contact you. That is a transfer of your information to them, and this notice is not telling you otherwise. It is done by hand, by a person, over the phone — this website sends nothing to Westminster Place automatically and has no connection to their systems.
What is never shared: your individual assessment answers, which never left your device, and any health information, because none was ever collected. Nothing is shared with anyone other than Westminster Place Admissions.
You can withdraw this permission at any time using the address below. If your details have already been passed to Admissions, withdrawing stops anything further here, but it cannot undo that call — you would need to ask Westminster Place directly to remove what they hold.
How long it is kept
The submission record is kept for 180 days from the date you submit it, then deleted automatically. This is enforced by the system itself rather than by someone remembering: expired records are removed whenever the form runs, and each run writes a dated receipt so the policy can be checked rather than taken on trust.
One honest limit, and it is a real one. The 180-day rule governs the record held by this site. When you submit, a notification email is also sent to the operator, and that email contains your name, phone number, email address and care level so he can make the call without a lookup. That message sits in his mailbox and with the email provider, Resend — neither of which the automatic 180-day deletion can reach. He clears it manually once the referral is made. If you ask to be removed, say so and the mailbox copy is deleted too.
A second honest limit. Once your details have been given to Westminster Place Admissions, what they do with them is governed by their own privacy practices, not by this notice. The 180-day rule deletes this site's copy. It has no power over theirs.
Asking to be removed
Write to norvenph@gmail.com and your record will be deleted. You do not need to give a reason, and you do not need your reference number — your name is enough. You can also ask what is held about you, and you will be sent it. Either request is answered within 30 days, and usually much sooner.
That address is read by the person who operates this guide. If your details have already been passed to Westminster Place Admissions, deleting the record here stops anything further from this site, but it cannot withdraw what they already hold — ask Westminster Place directly for that.
Children
This site is intended for adults making care decisions for an older relative. It is not directed at children and collects nothing from them.
